Care Coordinator – Young Community Developers

Under the direct supervision of the Program Manager and in close collaboration with key staff, the Care Coordinator is responsible for supporting, verifying, and processing clients in need of housing assistance. The Care Coordinator will work in-person with clients through the intake/application process to assess housing and other essential needs. The Care Coordinator will then work closely with clients demonstrating a qualifying housing need through the verification, approval, and funding process. In addition, maintaining detailed case records, funding requests, and reporting requirements.

Salary range: $24.00-$28.00 hourly

Project Overview
Committed to serving the District 10 and 11 in San Francisco, in an effort to address the housing crisis exacerbated by the COVID-19 pandemic, Young Community Developers (YCD) is looking to hire a Housing Care Coordinator to join the Housing Team to administer funding to help stabilize households throughout our community.

● Thoroughly and diligently, assess the client’s overall essential needs.
● Assist clients with identified housing needs through the intake, application, and verification process.
● Based on client assessment, make the appropriate referrals to other resources internally and externally.
● Maintaining a caseload with detailed records.
● Ability to provide services within the community with compassion and cultural competency while using a holistic approach to resolution.
● Maintain detailed and organized client case notes and files.
● Assisting in developing and streamlining processes as program changes and pivots.
● Working at both onsite and offsite offices.
● Tabling community events and community outreach.
● Working collectively and assisting other YCD programs and other community partners when needed.
● Data entry tasks using Salesforce, Microsoft Office Suite, Google workspace and other technology platforms.
● Ensures compliance with agency, funder, and governmental reporting requirements and supports all program evaluation efforts.
● Assists in preparation for program audits and monitoring visits.
● Participates in regular team, agency, funder, and community meetings.
● Other duties as assigned.

A successful candidate will possess:
● B.A./B.S. degree in a related field and at least two years of relevant experience preferred.
● Bilingual in English and Cantonese or English and Spanish will receive a bilingual pay differential of 8%.
● Ability to meet the needs of the community by working nontraditional hours as needed, which may include evenings and weekends.
● Excellent cross-cultural communication skills and the ability and willingness to work with economically and culturally diverse clientele from various professional and ethnic backgrounds.
● Excellent interpersonal and communication skills, written communications, patience, diplomacy, and high level of customer service skills.
● Ability to think critically, problem solve and be resourceful.
● Must have proven ability to work both independently and as part of a team.
● Must be comfortable with and adapt quickly to organizational, program and departmental changes.

● Well organized, ability to work in a fast-paced environment and meet goal objectives; excellent team player. True attention to detail, ability to complete paperwork accurately and in a timely manner.

LOCATION: San Francisco, CA
JOB TYPE: Full Time 40/hours – Non-exempt
COMPENSATION: $24.00-$28.00/hour DOE
